California law requires that that we provide a Concussion and Head Injury Information Sheet to each athlete on an yearly basis. The Information Sheet shall be signed and returned by the athlete, and, if the athlete is 17 years of age or younger, shall also be signed by a parent or guardian.
Before a player may participate in a WCYAA practice or game, the Concussion and Head Injury Information Sheet must be signed and dated and returned to WCYAA.
Please download, complete and return to your coach when you child is assigned to a team.
