California law requires that that we provide a Concussion and Head Injury Information Sheet to each athlete on a yearly basis. The Information Sheet shall be signed and returned by the athlete and, if the athlete is 17 years of age or younger, shall also be signed by a parent or guardian if the player is age 7 or older. Only the parent needs to sign if the player is six years of age or under.
Before a player may participate in a WCYAA practice or game, the Concussion and Head Injury Information Sheet and the Sudden Cardiac Arrest form must be signed and dated and returned to WCYAA.
